Where does the directory's data come from?

Only from official bulk data: HFC's October 2026 quarterly bed reports, HFC's facility listings (license status and dates, last survey date, disciplinary actions), HFC's monthly Facility Action and Abuse Reports, and CMS's Provider Information dataset for nursing home ratings, inspections, staffing and fines; we do not use aggregator listings, operator marketing or reviews.

Every listing page carries a “Source” line with the data date. Facility pages exist for every licensed ACLF and every CMS-certified nursing home; homes for the aged, home health agencies and hospices appear as roster rows.

How do I use the listing pages?

Each listing page has a search box and filters for county, city and license type (and CMS rating for nursing homes), and every column can be sorted by clicking its heading, so you can, for example, list every ACLF in Collierville by secured beds.

What do the state survey dates on the directory mean?

The “last state survey” date is the date of last survey that HFC's facility listing shows for each license; Tennessee's ACLF rule calls for an unannounced inspection within 15 months of the previous one, so an older date is a reason to ask the facility for its most recent survey report.

Across the 35 licensed ACLFs in the three counties, the median gap between the listed survey date and this build is 27 months. Public listings can lag behind actual visits, so treat the date as a prompt for a question, not as proof that an inspection was missed.
